Cogent Physical Rehabilitation Center

Cogent Physical Rehabilitation Center (“Cogent Rehab”) is a physiotherapy-led rehabilitation clinic in Burlington, Ontario, providing evidence-based, goal-oriented care for individuals recovering from injury, neurological events, and complex conditions that affect balance, mobility, and daily function.

Our practice supports clients across the continuum of recovery, with a particular focus on concussion and mild traumatic brain injury (mTBI), vestibular dysfunction (dizziness, vertigo, imbalance), and falls-risk management for older adults. We also provide comprehensive rehabilitation for individuals living with limb loss, including gait retraining, conditioning, balance retraining, functional mobility training, and return-to-community participation in collaboration with the client’s broader care team.

Cogent Rehab’s approach is grounded in thorough assessment, measurable outcomes, and individualized treatment planning. Care may include vestibular rehabilitation therapy (VRT), graded aerobic and functional exercise progressions, cervical and postural rehabilitation where indicated, balance and proprioceptive training, patient education, and self-management strategies that empower clients to understand symptoms and progress safely. We emphasize clear communication, compassionate care, and practical tools clients can use at home to support sustained improvement between visits.

We collaborate with family members, insurers, and community providers where appropriate, and we value timely documentation, functional goal-setting, and coordinated return-to-work/return-to-school planning. Our clinic environment is designed to be supportive and structured, with access to functional rehabilitation equipment that allows clients to rebuild capacity for real-world activities.

Cogent Rehab’s mission is to help people move from pain and limitations to meaningful function and independence. We welcome referrals and partnerships that improve access to high-quality rehabilitation for
